Online form to request, or decline, the flu jab.

Dear Parent/Carer,

The school-aged immunisation team is now inviting all secondary school pupils in years 7, 8 and 9 for their annual flu vaccination. The vaccination is free and is a quick and simple spray up their nose to help protect them against flu.

Flu can be an unpleasant illness and sometimes causes serious complications. Vaccinating your child also helps to protect vulnerable family members and friends by preventing the spread of flu.

Please visit the link below to access further information and complete your child’s consent form online.

If you would prefer a porcine-free injectable flu vaccination you can also choose this option by declining the flu spray and then consenting for the porcine-free vaccination

If you don’t want your child to receive any flu vaccination please complete the form declining this offer, as this will avoid further communications from our service.

If you are unable to complete the online form, please contact our team who will be happy to help on 0151 295 3833. If, after completing this form, you attend the GP or wish to amend the form you must contact the immunisation team directly and not your child’s school.

All Year 8 children are being offered the HPV vaccination which protects against a range of HPV related cancers. This is a two dose programme and when you complete the form, you are consenting to both doses. Could we kindly ask that all parents/carers access the below link

All year 9 children are offered vaccinations to boost their antibodies following the vaccines they received as a baby and ensure they remain protected against Diphtheria, tetanus, polio and Meningitis ACWY into adulthood.

Could we kindly ask that all parents/carers access the below link?

You can also access information and complete your child’s HPV consent form if they missed this in year 8, as its never too late to get this vital protection against HPV-related cancers.

Did your child miss any school vaccinations? It’s never too late to get protected.

All secondary school children are offered vaccinations to protect them against HPV-related cancers. As well as vaccinations to boost their antibodies following the vaccines they received as a baby and ensure they remain protected against Diphtheria, tetanus, polio and Meningitis ACWY into adulthood.

Please access the below link if you think your child has missed any routine vaccinations, including MMR vaccinations.
